PERMISSION AND PHOTO RELEASE

I give permission for my child(ren) to participate in the St. Ann Sunday School Program and its related activities during the 2026-2027 school year.

From time to time, we may take photos during classes or parish events to share the life of our parish community. I give permission for my child(ren)'s image to be used in parish and diocesan publications, on the parish website, and on the parish and diocesan social media. Children's names will not be published.

If there is ever an emergency, I understand the parish will make every effort to contact me using the information provided on this registration form.

A $40.00 registration fee per child is requested.

This year's registration fee has increased from $20 to $40 per child to help offset the rising cost of books, classroom supplies, and program materials, while also expanding the activities and events offered to our Religious Education families.

The registration fee helps cover:

Books, classroom supplies, and program materials
Christmas and commencement celebrations
Family gatherings and faith-building activities throughout the year, including a Family Afternoon, End-of-Year BBQ, and other special events.
